Record Transfer Form
At your pet’s first visit with us, we would encourage you to bring in any medical history that you may have. In addition, you can request that a copy of your pet’s medical records be transferred to us by completing the Medical Records Transfer Form and sending it to the previous veterinary hospital.
Prescription Refills
Thank you for your prescription refill request. In our ongoing effort to make your pet’s health care as convenient and easy as possible, you can now request a refill of your pet’s prescription or prescription diet by submitting the form below. Your pet must have been examined by Dr. Will within the past year to continue dispensing medications and prescription diets. Other restrictions do apply. Unless you are contacted by our office, your request will be ready for pick-up within 24 hours of placing this request. Please remember, you may also call our office directly at (716) 937-4588 during business hours with prescription requests or questions.
